PRN/Hourly | M-Sun 8a-430p Department/Unit Summary CaseManagement plays an important role in the care of hospitalizedpatients. Our team of RN Case Managers, Social Workers, UtilizationReview staff, and Discharge Planners assure that payer requirementsare met and develop an individualized plan of care for ourpatients. The Case Management staff coordinates with theinterdisciplinary team and community resources to maximize thepatient’s opportunity to succeed. Qualifications:

Qualifications and requirements Applicants shouldhave a current state RN license and possess an associates orbachelor’s degree from an accredited nursing school.

Additionalrequirements include:

• BLS Certification
• 3 years experience inacute care as a Professional Nurse or RN experience withsignificant acute care hospital Case Management experience
• Previous case management experience preferred
