On-CallResponsibilities are required, including some evenings and weekendson rotation with other Directors of Nursing.

Position Purpose:

Under the supervision of the Area Director of Clinical Services,this position exists to act as the supervising nurse to manage theclinical skilled home care staff and to provide the highest qualityof care and service for the patients in our Exton market. TheDirector of Nursing position will also work with other team membersto establish Care Options For Kids as the provider of choice forclients, caregivers, and referral sources in the region by managingstaff to provide excellent customer service, coordinating clientservices, and forging strong client and caregiver relationships.

Examples of Specific Areas of Responsibility to the

Position:
  • Assists with the planning, coordination, leading, controlling, andevaluating of home care services.
  • Leads the coordination andmanagement of home care clinical services.
  • Acts as a liaison withthe medical community, clients, and branch personnel.
  • Directs andsupervises professional and auxiliary personnel rendering clientcare services.
  • Ensures that the client’s plan of care is createdand executed as written.
  • Ensures that an appropriate health careprofessional performs an assessment on a client’s needs and areassessment when there is a significant health status change inthe client’s condition.
  • Develops standards that ensure thedelivery of safe and effective services to clients and families.
  • Plans, directs, and evaluates the implementation of service goalsand objectives.
  • Establishes and maintains effective channels ofcommunication.
  • Collaborates in the development and maintenance ofan effective and efficient client care documentation system.
  • Participates in selective committee meetings to promote complianceand efficiency.
  • Maintains confidentiality of medical records,personnel records, etc., within the scope of their position.
  • Works closely with the electronic medical record (EMR) to ensureclient electronic charts are maintained, updated, and remain incompliance with company standards.
  • Understands details around theknowledge of clinical workflow as it pertains to the use of EMR.
  • Participates in formal and informal problem-solving processes tomeet the EMR computer education and provides trainer sessions forpeers and field staff to expand training team expertise.
  • Providescoaching support to new staff to ensure compliance and consistencyas it relates to the EMR and conducts EMR chart audits.
  • Performsother duties as assigned in a professional matter.

  • Home Health Care:

    1 years (Required)

  • 1 years of supervisory experience in home health, administrative orequivalent experience is required

Annual Salary Range:
  • Full-Time, In Office:

    $100,000/year, with potential for bonus.

  • Compensation dependent onexperience.
