Responsibilities:

/n /n

  • Your role as the nursewill establish patients’ plans of care in collaboration withphysicians, patients, and utilization management (UM) professionalsto meet physicians’ orders and patients’ needs. /n
  • RN willcoordinate case management activities across a multi-disciplinaryhealth care team to ensure appropriate, quality patient care. /n
  • Initiate, deliver, and evaluate patient
  • and family-centered healthteaching. /n
  • Strive to help patients achieve optimal healthoutcomes, delivering compassionate care with an emphasis on thepatient’s experience. /n
  • Complete all required documentationincluding visit assessments, OASIS, 485 and verbal ordersaccurately and promptly on a laptop or in written documentationwhile in the patient’s home and according to all applicableregulatory standards. /n
  • Attend compliance training and adhere tothe organization’s standards of conduct, policies, and proceduresin addition to all local, state, and federal regulatory guidelines./n /n /n
  • The individual must be able to demonstrate the knowledgeand skills necessary to provide care and/or interact appropriatelyto the ages of the patients served by his/her assigned unit asspecified below.

They must also demonstrate knowledge of theprinciples of growth and development over the life span and possessthe ability to assess data reflective of the patients’ status andinterpret the appropriate information needed to identify eachpatients’ requirements relative to his/her age-specific needs andto provide the care needs as described in the department policy andprocedures. /n

  • Six (6) months of nursing experience in an AcuteCare or Skilled Care Facility or Home Health or Hospice Carepreferred. BSN preferred. /n
  • Ability to establish and maintainpositive, caring relationships with executives, managers,physicians, non-physician providers, ancillary and support staff,other departments, and patients/families.

Ability to workproductively and effectively within a complex environment, handlemultiple/changing priorities and specialized equipment. Goodclinical judgment with critical thinking, analytical andproblem-solving abilities required as related to various aspects ofpatient care. Critical thinking skills necessary to exercise and tolead others in application of the nursing process. Mobility andvisual manual dexterity. Physical stamina for frequent walking,standing, lifting, and positioning of patients. Successfulcompletion of Home Health/Hospice Orientation upon hire. /n /n

Licensure, Certifications, and Clearances:

/n /n

  • Graduate nursesmust complete licensure examination within SIX (6) MONTHS of hireor within one year of graduation, whichever comes first.

Thoughtemporary permits are valid for one year from the date ofgraduation, not one year from issue of permit (or until the resultsof the examination are known at which time it becomes null andvoid), UPMC requires that GNs take the licensure examination within6 months of their start date or they will be terminated or demotedfrom the GN position, at the discretion of the BU. /n

  • Basic Life Support (BLS) OR CardiopulmonaryResuscitation (CPR) /n

  • Registered Nurse(RN) OR Temporary Practice Permit (TPP) /n
  • Act 33 /n
  • Act 34 /n
  • OAPSA /n /n
  • Current licensure either in the state where thefacility is located or, if the facility is in a state covered bythe multistate Nursing Licensure Compact (NLC) agreement, amultistate license issued by a participating NLC state.
