Earn $45.02 per hour with full benefits or $47.02 per hour with the enhanced pay option. Includes mileage reimbursement.
Position DescriptionThe Registered Nurse (RN) is a vital member of the Assertive Community Treatment (ACT) team, providing rehabilitation, treatment, and crisis intervention under clinical supervision. This role focuses on delivering person-centered care, coordinating treatment activities, and offering supportive interventions to help individuals develop essential skills for independent living. The RN also plays a key role in medication management, assisting psychiatrists with administrative tasks, ensuring accurate medical documentation, and educating both clients and team members on medication use.
Key Responsibilities:
Community-Based Client Support (50-90% Travel). Provide direct care in clients’ homes and community settings, conducting health assessments, monitoring medication adherence, and addressing medical concerns.
Oversee the medication system, document treatments, educate clients and team members, and ensure compliance with prescribed regimens.
Work closely with medical providers, behavioral health professionals, and community resources to ensure integrated, comprehensive client care.
Participate in crisis response, support treatment planning, and provide health and wellness education to clients, their supports, and the ACT team.
The ACT team is a multidisciplinary group dedicated to supporting adults with serious mental illness (SMI) through community-based services, including therapy, medication management, and resource linkage. Services are primarily delivered in the community, with clients receiving at least three home visits per week. The RN contributes to a holistic approach to care, helping clients integrate into society while ensuring their basic needs and treatment goals are met. Frequent travel (50-90%) is required, and personal vehicle use for client transportation may be necessary.
