JOB-SPECIFIC

DUTIES/COMPETENCIESPerforms a nursing assessment on all patients within 8 hours ofadmission. The data collected forms the basis of an individualizedplan of care. Identifies teaching needs and goals related to thediagnosis of each patient from the nursing admission assessment andby collaborating with patient/family and the Interdisciplinaryteam. Provides patient information and status in a timely mannerfor the Initial Team Conference and for Re-evaluation TeamConferences on a weekly basis. Responsible and accountable foraccurate, succinct and timely documentation of all assessments,interventions and patient’s response in the medical record inaccordance with policy. Participates in the patient rehabilitationprogram on an assigned unit. Is responsible and accountable fordirect patient care by utilizing the nursing process of assessing,planning, implementing and evaluating patient care with a priorityon the education of the patient/family/significant other based onthe health care needs identified. Uses a positive and encouragingapproach to achieve patient goals and quality outcomes. Initiatesdischarge planning on all patients from the day of admission.

Initiates patient teaching in anticipation of the discharge date.

Communicates all pertinent information regarding changes inpatient’s status from one care provider to another, to supportcontinuity of care and patient safety. This includes the attendingphysician and the Charge Nurse. Seeks consultation from more seniornursing staff in support of complex clinical decision-making.

Provides direction, training and supervision of ancillary nursingpersonnel by utilizing problem solving techniques in decisionmaking, organizing, setting priorities and using time effectively.

Delegates appropriately. Provides for and assures personal andenvironmental safety in accordance with hospital and nursingpolicies and procedures. Positively contributes to the functioningof the unit by providing calm direction and reassurance toancillary staff, other departments, patients and families.

Participates in team building activities on the unit to create apositive and efficient work environment. Maintains communicationwith manager and senior nursing staff related to service needs suchas necessary supplies, equipment (including repairs) and clinicaltraining in order to effectively complete assigned duties. Providescare in a cost effective manner while promoting quality patientoutcomes, monitors supplies and charges. Assumes chargeresponsibility following orientation period as requested.

Identifies and initiates own professional growth, actively pursuingprofessional development through continuing education and selfstudy. Prioritizes activities and plans for effective use of timeand staff in meeting patient needs. Performs other duties asrequested.
