The NP will play a key role in delivering high-quality medical care directly to patients in their homes, with a strong focus on chronic disease management, functional assessments, and reducing preventable hospitalizations. This role is ideal for a provider who values personalized care, clinical autonomy, and making a direct impact in patients’ lives. Key Responsibilities Perform comprehensive in-home assessments of adult and geriatric patients, including history, physical exams, medication reviews, and functional evaluations Develop and manage individualized treatment and care plans Monitor chronic conditions such as hypertension, diabetes, COPD, and heart failure, with proactive interventions Prescribe and manage medications, diagnostic testing, and durable medical equipment as needed Coordinate care with collaborating physicians, home health agencies, caregivers, and specialists Provide patient and caregiver education on disease prevention, medication use, and wellness strategies Maintain accurate, timely, and thorough documentation in the electronic health record (EHR) Recognize and manage acute issues in the home setting to prevent emergency room visits and hospital admissions Participate in quality improvement initiatives and case conferences as needed Qualifications & Skills Current Nurse Practitioner license and certification (FNP or AGNP preferred) Active DEA registration (or eligibility to obtain) At least 1 year of clinical experience in primary care, geriatrics, or home-based care Proficiency with EHR systems and mobile documentation Strong clinical judgment, time management, and communication skills Comfortable working independently in home environments Valid driver’s license and reliable transportation Preferred Experience with value-based care models or home health Knowledge of transitional care, palliative care, or chronic care management (CCM)
