You’ll operate independently in the field while staying closely connected with a supportive, mission-driven clinical team. Responsibilities

• Conduct in-home health assessments and preventative services
• Create personalized care plans focused on prevention & chronic conditions
• Provide screenings, geriatric evaluations, and wound care (a plus)
• Educate patients and families on wellness and healthy living
• Collaborate with care teams and maintain accurate records Qualifications
• NP certification (required)
• Experience with home-based care and geriatric patients (preferred)
• Wound care skills a plus
• Excellent communication and independence
• Reliable transportation

Full-time Pay:

$140,000.00 – $145,000.00 per year
