As part of a supportive and collaborative nursing team, you’ll play a vital role in Transitional Care Management and lead efforts in quality improvement initiatives. Your work will directly support primary care practices and providers, with minimal travel needed to maintain strong, local connections. Preferred candidates will bring experience in telephonic patient outreach, EPIC, and managing chronic conditions within a primary care setting. Responsibilities:

Collect, prepare, and present accurate, timely data to display results of process improvements that allow for trending and are easy to understand.
Review data and analyze outcomes for patterns and trends in order to communicate findings related to standards, evidenced based practices, and policies for the improvement of processes.
Utilize appropriate computer systems to obtain data, enter data, and develop reporting tools.

Qualifications:

Required:

Bachelor’s degree in Nursing or related field required, either at time of hire/transfer, OR must be enrolled in a program within one year of hire/transfer date and completed within three years of enrollment.
Minimum of two (2) years of nursing experience required.
Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients and families.
Ability to work productively and effectively within a complex environment, handle multiple and changing priorities and specialized equipment.
Exemplary critical thinking, analytical and problem solving abilities required as related to various aspects of patient care.
Critical thinking skills necessary to exercise and to lead others.
Mobility and visual manual dexterity.
Physical stamina for frequent walking, standing, lifting.
