UPMC’s Physician Network Administration in Erie, PA is excited to invite qualified candidates to for our full-time Quality Nurse position. This opportunity offers a consistent Monday through Friday schedule from 8:00 AM to 4:30 PM, with no weekends or holidays required, allowing you to focus on both your professional and personal goals. As part of a supportive and collaborative nursing team, you’ll play a vital role in Transitional Care Management and lead efforts in quality improvement initiatives. Your work will directly support primary care practices and providers, with minimal travel needed to maintain strong, local connections. Preferred candidates will bring experience in telephonic patient outreach, EPIC, and managing chronic conditions within a primary care setting. Responsibilities:

Review and extract patient information from the medical record.
Participate in quality projects.
Aggregate and report data based on project requirements.
Investigate and summarize key issues based on quality case project.
Understand and maintain knowledge of regulatory and/or standards by actively reviewing and updating process to reflect the requirements.
Promote change and innovation that enhances the quality of patient care and the departmental functions and efficiencies.
Demonstrate a passion for developing others, enthusiastically and expertly precept students, graduate nurses, and new hires.
Analyze clinical and behavioral situations, identifying learning opportunities to improve patient care practices.
Regularly seek feedback on performance and continuous professional improvement.
Collect, prepare, and present accurate, timely data to display results of process improvements that allow for trending and are easy to understand.
Review data and analyze outcomes for patterns and trends in order to communicate findings related to standards, evidenced based practices, and policies for the improvement of processes.
Utilize appropriate computer systems to obtain data, enter data, and develop reporting tools.

Qualifications:
Required:

Bachelor’s degree in Nursing or related field required, either at time of hire/transfer, OR must be enrolled in a program within one year of hire/transfer date and completed within three years of enrollment.
Minimum of two (2) years of nursing experience required.
Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients and families.
Ability to work productively and effectively within a complex environment, handle multiple and changing priorities and specialized equipment.
Exemplary critical thinking, analytical and problem solving abilities required as related to various aspects of patient care.
Critical thinking skills necessary to exercise and to lead others.
Mobility and visual manual dexterity.
Physical stamina for frequent walking, standing, lifting.

Licensure, Certifications, and Clearances:

Registered Nurse (RN)
Act 33 with renewal
Act 34 with renewal
Act 73 FBI Clearance with renewal •Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to for licensure within that state.
