Full-time |M-F 8a-430pm, rotating Saturdays and On-callDepartment/Unit Summary Case Management plays an important role inthe care of hospitalized patients. Our team of RN Case Managers,Social Workers, Utilization Review staff, and Discharge Plannersassure that payer requirements are met and develop anindividualized plan of care for our patients. The Case Managementstaff coordinates with the interdisciplinary team and communityresources to maximize the patient’s opportunity to succeed. Qualifications:

Qualifications andrequirements Applicants should have a current state RN license andpossess an associates or bachelor’s degree from an accreditednursing school.

Additional requirements include:

• BLSCertification
• 3 years experience in acute care as a ProfessionalNurse or RN experience with significant acute care hospital CaseManagement experience
• Previous case management experiencepreferred
• Case management certification preferred
