Responsibilities:
As a Registered Nurse Case Manager (RNCM), you will have the following responsibilities: Provide professional nursing care, complete admission, initial comprehensive, and ongoing assessments pf patients. Initiate, develop and update individual appropriate plan of care according to an individual patient’s needs. Working collaboratively with the hospice physician and Interdisciplinary Team to carry out appropriate interventions. Managing and supervising the combined efforts of home health aides, social workers, and chaplains. Educate patients and caregivers as appropriate. Upholding high standards of compliance regarding documentation and patient care requirements. Helping the agency to continuously increase the quality of care.

Qualifications:
Current PA Registered Nurse (RN) License Experience in Nursing Willing to travel to the assigned areas Must have a valid drivers’ license with reliable transportation and liability insurance Strong communication and interpersonal skills If you meet these qualifications, we want to meet you!
