PURPOSEAND SCOPE
This position is accountable and responsible for theprovision and coordination of clinically competent care includingassessment, planning, intervention and evaluation for an assignedgroup of patients. This may include delegation of appropriate tasksto direct patient care staff including but not limited to RNs,LVN/LPNs and Patient Care Technicians. As a member of the End StageKidney Disease (ESKD) health care team, this position participatesin decision-making, teaching, leadership functions, and qualityimprovement activities that enhance patient care outcomes andfacility operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Allduties and responsibilities are expected to be performed inaccordance with Client policy, procedures, standards of nursingpractice, state and federal regulations. Performs all essentialfunctions under the direction of the Supervisor and with guidancefrom the Clinical Educator, Preceptor or in collaboration withother Registered Nurses. Performs ongoing, systematic collectionand analysis of patient data pre

- post hemodialysistreatment for assigned patients and documents in the patientmedical record, makes adjustments or modifications to treatmentplan as indicated and notifies Team Leader, Charge Nurse,Supervisor or Physician as needed.
Assesses, collaborates, anddocuments patient/family’s basic learning needs to provide initialand ongoing education to patients and family. Directs and provides,in collaboration with direct and ancillary patient care staff, allaspects of the daily provision of safe and effective delivery ofhemodialysis therapy to assigned patients. Administers medicationsas prescribed or in accordance with approved algorithm(s), anddocuments appropriate medical justification and effectiveness.
Initiates or assists with emergency response measures. Serves as aresource for health care team, participates in staff training andorientation of new staff as assigned. Ensures correct laboratorycollection, processing and shipping procedures are performed andreschedules missed or insufficient laboratory collections.
Identifies expected outcomes, documents and updates the nursingassessment and plan of care for assigned patients throughcollaboration with the Interdisciplinary Team. Ensures patientawareness related to transplant and treatment modality options.

PHYSICALDEMANDS AND WORKING CONDITIONS
The physical demands and workenvironment characteristics described here are representative ofthose an employee encounters while performing the essentialfunctions of this job. The position provides direct patient carethat regularly involves heavy lifting, moving of patients andassisting with ambulation. Equipment aids and/or coworkers mayprovide assistance. This position requires frequent, prolongedperiods of standing and the employee must be able to bend over. Theemployee may occasionally be required to move, with assistance,machines and equipment of up to 200 lbs., and may lift chemical andwater solutions of up to 30 lbs. as high as 5 feet. The workenvironment is characteristic of a health care facility with airtemperature control and moderate noise levels. May be exposed toinfectious and contagious diseases/materials. Day to day workincludes desk work, computer work, interaction with patients,facility/hospital staff and physicians. The position may requiretravel to training sites or other facilities. May be asked toprovide essential functions of this position in other locationswith the same physical demands and working conditions as describedabove. Candidates are required to take the Ishihara’s ColorBlindness test as a condition of employment.

SUPERVISION
Assigned oversight of Patient CareTechnicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse inCharge, after meeting all the following: Successful completion ofall client education and training requirements for new employees.
Must have a minimum of 9 months experience as a RN. Must have aminimum of 3 months experience in chronic/acute hemodialysis as aRN. EDUCATION and
LICENSURE
Graduate of an accredited School ofNursing. Current appropriate state licensure. Current or successfulcompletion of CPR BLS Certification. Must meet the practicerequirements in the state in which he or she is employed.
EXPERIENCE AND REQUIRED SKILLS
1 year of Nephrology Nursingexperience in the last two years as a RN.
Chronic hemodialysisexperience required Floating requirement:
Client requiredcandidates to float to area sites within 50 miles of the main siteShift details: Shifts will be discussed in detail during theinterview; the role requires a commitment of 40 hours plus overtimeand floating to area sites as need be. There is a minimum guaranteeof 36 hours per week. Please note you are NOT eligible forGuaranteed hours if any of the following happen: /t
