Responsibilities:

Follow a designated care plan in accordance withpatient’s needs Make home visits to clients in designatedgeographic territories Perform assigned duties, includingadministration of medication, wound care, treatments, andprocedures Monitor clients’ conditions; reporting changes toClinical or Client Services Manager Follow up with, execute, andproperly document doctors’ orders Perform client assessments asnecessary Case management and coordination Accurately documentobservations, interventions, and evaluations pertaining to clientcare management and services provided, utilizing a state-of-the-arttouch pad tablet Qualifications for a Registered Nurse (RN): Acurrent license as a Registered Nurse in PA. A minimum of one yearof recent, verifiable clinical (medical/surgical) experience. Priorhome care experience strongly preferred, but not required.

Graduation from an accredited and approved nursing program, asindicated by school transcript or diploma. Ability to workindependently and manage time effectively. Strong interpersonalskills. Solid computer skills; prior experience with electronicmedical records (EMR) preferred. Ability to travel to cases asassigned.
