Key Responsibilities:
Coordinate the development and completion of resident assessments (MDS) in accordance with all regulations, including RAPs and Triggers Maintain and update policies related to MDS and care planning Participate in facility surveys conducted by authorized agencies Evaluate resident conditions and medical data to assess care needs Partner with the DON to notify the care team of new admissions Coordinate with external care plan team members as needed Develop preliminary and comprehensive nursing assessments using required forms Ensure accuracy and compliance of the assessment team’s work and promote awareness of regulatory standards
Qualifications:
Active RN license in the state of Pennsylvania Minimum 2 years of RNAC experience in a long-term care setting Strong knowledge of managed care and bundled payment systems Hands-on experience with PDPM and MDS Excellent communication skills and computer proficiency Deep understanding of state and federal regulations and the survey process Ability to provide training, oversight, and corrective action when needed We are proud to be an Equal Employment Opportunity employer and welcome applicants of all backgrounds.
