Position Overview:

We are seeking a full-time Home Health and Hospice Community Liaison to join our growing team. This position is ideal for a dynamic, self-motivated professional who enjoys being on-the-go and has a passion for making meaningful connections. The Community Liaison will be responsible for developing and implementing effective marketing strategies, building strong relationships with healthcare providers, community partners, and referral sources to promote our Home Health and Hospice services.

Key Responsibilities:

Develop and execute an effective marketing plan to increase referrals for home health and hospice services. Educate healthcare professionals, facilities, physicians, and community organizations on the benefits and services of Advanced Home Health and Hospice. Promote agency goals, mission, and values to community partners. Build and maintain strong professional relationships with referral sources. Identify new business opportunities and cultivate strategic partnerships. Collaborate with clinical leadership to ensure seamless patient transitions and quality care delivery. Attend community events, health fairs, and professional networking events to promote agency services. Provide timely and accurate reporting on marketing activities and referral trends. Remain active and on-the-go throughout the day visiting referral sources, facilities, and community partners.

Requirements:

Minimum of 2 years of experience in a medical setting (required). Experience working with elderly patients (preferred) Knowledge of medical terminology (preferred). Previous experience in healthcare sales, marketing, or community liaison roles is highly desirable Strong interpersonal, communication, and relationship-building skills. Self-starter with excellent organizational and time-management abilities. Ability to work independently while being part of a collaborative team. Valid driver’s license, reliable transportation, and current auto insurance.

Schedule:

Day shift Monday to Friday Weekends as needed
