Nurse Practitioner•Homecare Transitional Care Provider Join our dynamic healthcare team as a Nurse Practitioner specializing in Transitional Care at Home. Job Requirements
Active, non-probationary state medical license
1+ years of Advanced Practice Nurse (APN) experience, preferably with a Medicare population
Experience in home-based care (as RN or APP) preferred
Flexibility to travel throughout the service area
Proficiency with Electronic Medical Records
Strong computer skills, including Gmail, MS Word, Excel, etc.
Additional language proficiency in Spanish, Polish, Russian, or other languages spoken within the communities we serve preferred but not required
Responsibilities
Provide post-discharge support within the patient’s home
Develop care plans and individualize goals of care with patients and their families
Optimize and monitor clinical status, address gaps in care, and reconcile medications
Collaborate with Transitional Care Managers, home health, social workers, hospitals, SNFs, and specialists
Recognize, diagnose, and manage acute and chronic medical conditions
Provide after-hours and weekend call support, shared with other in-state providers
Facilitate goals of care and advance care planning discussions
Assess and evaluate family/caregiver needs and limitations
Teach patients and caregivers about health conditions
Ensure accurate and timely documentation of patient encounters
