$58,000-$68,000 yearly Position Summary The Central Admissions Nurse is responsible for reviewing and processing clinical referrals for skilled nursing and rehabilitation admissions across multiple facilities. This role ensures that each referral meets clinical criteria and is appropriate for placement within the organization’s capabilities. The position also serves as a liaison with hospitals, discharge planners, and internal admissions teams, supporting census development. Occasional external sales and marketing efforts may be required to support strategic growth initiatives.

Key Responsibilities Clinical Review & Referral Processing:

Review clinical documentation for incoming referrals to determine if patient needs can be met by the receiving facility. Collaborate with facility clinical teams, admissions coordinators, and leadership to evaluate admission eligibility. Communicate acceptance, denial, or need for additional information to referral sources. Accurately document all referral decisions and reasons in the tracking system or CRM. Coordination with

Maintain timely, professional communication with hospital case managers, discharge planners, and physicians. Build and strengthen referral relationships through responsiveness and expertise. Sales & Marketing Support (As Needed): Participate in hospital visits, community outreach events, or meetings with referral partners. Support the sales team by representing the organization’s clinical capabilities and strengths.

Documentation & Reporting:

Maintain accurate referral logs and EMR/CRM entries. Track acceptance rates, denial reasons, and trends for reporting purposes. Ensure documentation is HIPAA-compliant and aligns with company policies. Qualifications Licensed Practical Nurse (LPN) in good standing, required. Minimum 2 years of experience in skilled nursing, rehab, or hospital discharge planning. Strong knowledge of post-acute care criteria, including Medicare, Medicaid, and managed care plans. Excellent clinical judgment and communication skills. Experience with EMR systems (e.g., PointClickCare) and referral platforms (e.g., naviHealth, CarePort) preferred. Ability to work in a fast-paced, multi-facility environment. Schedule & Travel Full-time, typically Monday-Friday some remote occasional weekend required.
