Job Summary:

We are seeking an experienced and compassionate RN Case Manager – Hospice to join our team. This role involves delivering high-quality, patient-centered hospice care, managing a caseload primarily in Lancaster County with potential support in York County. The office is based in York County.

Key Responsibilities:

Perform comprehensive assessments of high-risk hospice patients. Develop and implement individualized care plans in collaboration with primary care providers. Complete admission, recertification, routine, PRN, and death visits as required. Maintain productivity benchmark of 5 to 7 visits per day. Coordinate non-medical support services (e.g., housing, transportation) to enhance compliance with treatment plans. Support care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals. Utilize specialty resources to ensure optimal patient outcomes. Keep thorough documentation of clinical, functional, and financial outcomes. Identify opportunities for health promotion and illness prevention. Prevent or minimize adverse events through proactive intervention. Serve as a patient advocate, protecting patient privacy and confidentiality.

Requirements:

Associate’s or Bachelor’s Degree in Nursing or a related field. Active, unrestricted RN license in the state of Pennsylvania. Valid driver’s license, car insurance, and BLS/CPR certification required. Hospice and HomeCare HomeBase (HCHB) experience required. Ability to analyze complex medical information and make sound decisions. Case Management certification preferred. #non
