Director of Clinical Services Job Summary The Home Health RN•Weekend provides skilled nursing care and case management to patients in the home setting. This full-time position includes 36 hours weekly, scheduled Friday through Sunday from 8:00 a.m. to 8:00 p.m. The RN works independently to assess, plan, implement, and evaluate patient care and serves as a vital member of the interdisciplinary team. Care is delivered in accordance with Medicare Conditions of Participation, PA licensure laws, and agency standards. Primary Responsibilities Clinical Care & Case Management Perform comprehensive nursing assessments and develop individualized plans of care. Provide skilled nursing interventions and evaluate outcomes of care. Collaborate with physicians, therapists, aides, and family members to ensure optimal patient outcomes. Accurately document all services provided and maintain timely clinical records. Educate patients and caregivers on disease processes, medications, and self-care techniques. Supervise Home Health Aides in compliance with Medicare requirements. Attend and participate in interdisciplinary team meetings and case conferences. Maintain productivity expectations based on patient acuity. Participate in on-call rotation as required. Care Coordination Manage daily assignments for maximum efficiency and effectiveness. Ensure proper communication with internal teams, physicians, and referral sources. Demonstrate accountability in coordinating care that meets agency standards and patient needs. Secondary Responsibilities Participate in staff meetings, committees, and educational in-services. Support the Home Health department during periods of high census or staffing needs. Maintain compliance with agency policies and federal/state regulations. Perform other duties as assigned. Physical Requirements and Working Conditions Strong verbal and written communication skills. Ability to work in diverse home environments. Valid PA driver’s license and reliable transportation required. Ability to lift up to 25 lbs and travel between patient homes. Qualifications Graduate of an accredited Registered Nursing program; BSN preferred. Minimum 1 year of clinical nursing experience; Home Health, Med/Surg, or Critical Care preferred. Current RN licensure in the Commonwealth of Pennsylvania. Current CPR certification. Proficiency in clinical documentation and care coordination. Satisfactory background checks and professional references.
