TheOncology Nurse Navigator (RN) is responsible for partnering withpatients and providers to offer personalized, individualized careof patients throughout the continuum of care to increase and assuretimely access to healthcare services. This role will support theHartford Region of Hartford Healthcare. Key Responsibilitiesinclude but are not limited to:
- Serves as a member of themultidisciplinary care team, functioning as a patient advocate,educator, key point of contact and liaison between the patient andall other appropriate services.
- Navigates patients through thecontinuum to eliminate barriers to health care and enhancingpatient and provider satisfaction.
- Functions as a collaboratingmember of the health care team, but also must be able to actindependently to assess patient and provider needs to ensureappropriate handoffs so that patients receive services promptly andefficiently.
- Applies the nursing process as appropriate withinthe context of the organization’s framework for professionalpractice and in accordance with guidelines established by the team.
- Assess patients for psychological distress and make appropriatereferrals if more intensive support is needed.
- Activelyparticipates in professional meetings and conferences i.e. diseasemanagement teams, cancer conferences, and nursing meetings.
- Ability to effectively and efficiently coordinate, implement, andmonitor the patient’s process of care, to communicate openly andempathetically with patients and their family members
- Ability towork independently as well as collaboratively with navigation teamand multidisciplinary care team in all aspects of defining andimplementing navigation program.
- Ability to communicateeffectively with diverse audiences including professional staff,patients, community, media, and other service agencies.
- Participates as needed in community education outreach programs/workshops/seminars/support services.
Qualifications Education :
- BS in Nursing strongly preferred;
Bachelor’s Degree RequiredLicenses/Certifications :
- CT RN license required.
Certificationin specialty area preferred
Experience:
- Minimum 5 years clinicalnursing experience preferred
- Demonstrates use of clinical skillsand expertise in the area of diagnosis, treatment, and psychosocialimpact of disease.
- Demonstrates excellent customer service skillsincluding superior telephonic communications skills with theability to comprehensively assess patient’s physical andpsychosocial status as well as conduct complex medical historydiscussions over the telephone.
- Knowledge of adult learningcompetencies and patient teaching skills
- Demonstrated managementskills reflecting self-motivation, initiative and fiscalresponsibility We take great care of careers.
