The Oncology Nurse Navigator(RN) is responsible for partnering with patients and providers tooffer personalized, individualized care of patients throughout thecontinuum of care to increase and assure timely access tohealthcare services. This role will support the Hartford Region ofHartford Healthcare. Key Responsibilities include but are notlimited to:

  • Serves as a member of the multidisciplinary careteam, functioning as a patient advocate, educator, key point ofcontact and liaison between the patient and all other appropriateservices.
  • Navigates patients through the continuum to eliminatebarriers to health care and enhancing patient and providersatisfaction.
  • Functions as a collaborating member of the healthcare team, but also must be able to act independently to assesspatient and provider needs to ensure appropriate handoffs so thatpatients receive services promptly and efficiently.
  • Applies thenursing process as appropriate within the context of theorganization’s framework for professional practice and inaccordance with guidelines established by the team.
  • Assesspatients for psychological distress and make appropriate referralsif more intensive support is needed.
  • Actively participates inprofessional meetings and conferences i.e. disease managementteams, cancer conferences, and nursing meetings.
  • Ability toeffectively and efficiently coordinate, implement, and monitor thepatient’s process of care, to communicate openly and empatheticallywith patients and their family members
  • Ability to workindependently as well as collaboratively with navigation team andmultidisciplinary care team in all aspects of defining andimplementing navigation program.
  • Ability to communicateeffectively with diverse audiences including professional staff,patients, community, media, and other service agencies.
  • Participates as needed in community education outreach programs/workshops/seminars/support services.
Qualifications Education :
  • BS in Nursing strongly preferred;
Bachelor’s Degree RequiredLicenses/Certifications :
  • CT RN license required.

Certificationin specialty area preferred

Experience:
  • Minimum 5 years clinicalnursing experience preferred
  • Demonstrates use of clinical skillsand expertise in the area of diagnosis, treatment, and psychosocialimpact of disease.
  • Demonstrates excellent customer service skillsincluding superior telephonic communications skills with theability to comprehensively assess patient’s physical andpsychosocial status as well as conduct complex medical historydiscussions over the telephone.
  • Knowledge of adult learningcompetencies and patient teaching skills
  • Demonstrated managementskills reflecting self-motivation, initiative and fiscalresponsibility We take great care of careers.
