Responsibilities include:

Perform clinical assessments of new and current clients in order to develop thorough and accurate care plans in accordance with state regulations and company policies. Revise care plans as necessary. Complete service agreements with clients as necessary Manage, prioritize and assign case load with Assessment nurses Ensure care plans and caregiver medical forms are accurately maintained in CRM system and client files Assign tasks and track adherence in CRM system so caregivers can report daily activity Consults with client, family and Caregivers to provide a safe and efficient patient care and review progress Supervise and evaluate performance of CNAs based on care provided to client and family. On-call phone for clinical emergencies. While office staff is primary contact for on-call, be available to assist caregivers and clients if there are clinical emergencies after hours Direct all personal care client services as per company scope of practice and provide overall coordination with office staff (Office Manager, Scheduler, Recruiter, Client Care Coordinator, Admin, etc) Participate in staff meetings, track assessments and report on all pertinent issues in a timely manner Qualifications include: Bilingual (English & Spanish) 2 years’ experience in home care, hospice, visiting nurse or community health experience is preferred Excellent written and verbal communicator with strong interpersonal skills Ability to work in a collaborative environment Excellent computer skills Must be self-motivated, organized, and able to work independently Has a valid driver’s license and a reliable car, willing to travel to clients’ homes
