Job Summary:

The registered Nurse creates individual care plan and performs supervisory visits in the client’s homes. At each home visits the RN supervises the aide and evaluates the status of the client. The RN works under the direction of the Director of nursing and the CEO.

Skills & Qualifications:

RN with an active RI license Home Care experience is a plus. Strong organizational skills and ability to work independently Excellent communication skills, both written and verbal. Spanish speaking a plus.

Responsibilities:

Make the initial evaluation visit and regularly reevaluate the client’s needs. Initiates the plan of care and necessary revisions every 60 days. Completes Minimum Data Set-Home Care (MDS-HC) and agency paperwork. Prepares progress notes for each client visit and summaries of care conferences on his/her clients in a timely manner per agency policy. Informs personnel of changes in condition and needs of the client. Counsels the client and responsible party/legal representative in meeting goals and related needs. Understands and adheres to established agency policies and procedures. Participates in peer review and Quality Assessment and Performance Improvement as assigned. Completes and submits assessments, supervisory visits, reassessments, resumption of care, change in program and significant change in condition in accordance with agency defined time frames. Supervises CNA and homemakers on the job. Evaluates CNA and homemakers per policy and procedure. Helps plan in-service training. Be available to answer clinical questions as needed by the filed staff and office staff. Review new employee medical information Cover for other RN’s when they are out for an extended time. i.e. pick up more visits per month. Contribute to the agency by facilitating relationships between referral sources, clients, families, and staff. Willing to travel and work in Providence and surrounding cities and towns.

Work Schedule & Environment:

Flexible to create your own schedule/workdays. No nights, No on-call, occasional weekend visits. Travel in personal vehicle to Providence and surrounding cities/towns. Report to agency office weekly.

Pay:

$40.00 – $50.00 per visit.

30-35 per week
