We are seeking a Supervisor of Care Navigation to join our Department of Care Coordination at Rhode Island Hospital in Providence, RI. In this full-time role, you will assist the Manager of Care Navigation in performing supervisory duties in support of all care navigation programs for Community Paramedicine, Outpatient High Risk Case Management, Surgical Services Case Management, and Nurse Care Navigators.

Responsibilities:

Identify and review potential patient candidates for the Community Paramedicine program Collaborate with outpatient providers and services to guarantee appropriate follow-up for community patients Conduct interviews with patients in the Surgical Services program and organize home care services prior to their surgery Monitor employee performance through chart audits and scheduling, and address any barriers or concerns for patients in all programs Gather and maintain data for each program and follow up with stakeholders as needed Offer case management consultation within the hospital and externally Organize paramedicine services, referrals, and discharge evaluations Engage in system-wide 24/7 support for clinical and scheduling matters Provide supervisory support for case management supervisors when required Assist with case management staff onboarding as necessary

Qualifications:

BSN Required Current RN license in RI (or a state that participates in the Nurse Licensure Compact) 3+ years of clinical case management experience preferred Experience supervising a team of up to six full-time employees Demonstrated independent decision-making skills, critical analytical skills, and excellent interpersonal skills Proven knowledge and skills necessary for providing patient care and managing care services, considering human development stages and cultural patterns Working at Brown University Health As of October 15, 2024, Lifespan is now Brown University Health.
