RequiredQualifications:

One year of recent experience as an RN Case ManagerValid RN license in compliance with state regulations Current BLS(AHA/ ARC)

Summary:

The Case Manager RN coordinatesand facilitates patient care across the healthcare continuum,ensuring optimal use of resources and improved patient outcomes.

They assess patient needs, develop comprehensive care plans, andcollaborate with multidisciplinary teams to provide efficient,cost-effective, and high-quality patient care.

Essential WorkFunctions:

Assess patients’ medical, psychosocial, and functionalneeds Collaborate with interdisciplinary teams, includingphysicians, therapists, social workers, and case managers, todevelop patient-centered care plans Develop and updateindividualized care plans, aligning with patient goals and clinicalguidelines Coordinate transitions of care ensuring seamlessservices and resources across the continuum of care Facilitatecommunication between patients, families, and interdisciplinaryteam members Monitor and evaluate patient outcomes, and adjust careplans as needed Ensure compliance with regulatory standards andinsurance requirements Advocate for patients’ preferencesaddressing barriers to care and promoting equitable access toservices Conduct utilization reviews and manage length of stayensuring adherence to evidence-based guidelines for care deliveryEducate patients and families on health conditions and availablecommunity resources Document case management comprehensively andaccurately in the patient’s medical record Participates ininterdisciplinary team meetings Perform other duties as assignedwithin scope Adhere to hospital safety protocols, infection controlguidelines, and regulatory standards

Required Essential Skills:

Critical thinking, service excellence and good interpersonalcommunication skills The ability to read, write, & communicate inthe English language is required Ability to read/comprehend writteninstructions, follow verbal instructions, and proficiency in PCskills Physical Abilities

• Remain in a stationary position, moveabout, move equipment (50-100lbs), pushing, pulling, bendingInterpersonal Skills
• To work effectively with a variety ofpersonnel (professional and ancillary) to present a positiveattitude and professionalism Technical/Motor Skills
• Ability tograsp, fine manipulation, pushing/pulling and able to move aboutwhen assisting with procedures and/or using department equipment.

Mental Requirements

• Must be able to cope with frequent contactwith the general public, customers, and meeting deadlines underpressure. Must be able to work under close supervision occasionallyas well as working without assistance from other personnel.
