Required Qualifications:

One year of recent experience as anRN Case Manager Valid RN license in compliance with stateregulations Current BLS (AHA/ ARC)

Certification PreferredQualifications:

ACLS Certifications (AHA/ARC) Other certificationsand licenses may be required for this position

Summary:

The CaseManager RN coordinates and facilitates patient care across thehealthcare continuum, ensuring optimal use of resources andimproved patient outcomes. They assess patient needs, developcomprehensive care plans, and collaborate with multidisciplinaryteams to provide efficient, cost-effective, and high-qualitypatient care.

Essential Work Functions:

Assess patients’ medical,psychosocial, and functional needs Collaborate withinterdisciplinary teams, including physicians, therapists, socialworkers, and case managers, to develop patient-centered care plansDevelop and update individualized care plans, aligning with patientgoals and clinical guidelines Coordinate transitions of careensuring seamless services and resources across the continuum ofcare Facilitate communication between patients, families, andinterdisciplinary team members Monitor and evaluate patientoutcomes, and adjust care plans as needed Ensure compliance withregulatory standards and insurance requirements Advocate forpatients’ preferences addressing barriers to care and promotingequitable access to services Conduct utilization reviews and managelength of stay ensuring adherence to evidence-based guidelines forcare delivery Educate patients and families on health conditionsand available community resources Document case managementcomprehensively and accurately in the patient’s medical recordParticipates in interdisciplinary team meetings Perform otherduties as assigned within scope Adhere to hospital safetyprotocols, infection control guidelines, and regulatory standards

Required Essential Skills:

Critical thinking, service excellenceand good interpersonal communication skills The ability to read,write, & communicate in the English language is required Ability toread/comprehend written instructions, follow verbal instructions,and proficiency in PC skills Physical Abilities

  • Remain in astationary position, move about, move equipment (50-100lbs),pushing, pulling, bending Interpersonal Skills
  • To workeffectively with a variety of personnel (professional andancillary) to present a positive attitude and professionalismTechnical/Motor Skills
  • Ability to grasp, fine manipulation,pushing/pulling and able to move about when assisting withprocedures and/or using department equipment. Mental Requirements
  • Must be able to cope with frequent contact with the general public,customers, and meeting deadlines under pressure.
