• Up to 6 weeks (including Holidays) Responsibilities In addition to performing visits and completing coordination ofclient care, the Registered Nurse (RN) Case Manager is the point ofcontact for all disciplines involved with providing care topatients and oversees the frequency of visits for the episode.
• TheRN Case Manager consults as needed
• with the physician and theoffice giving details about patient care.
• Performing accurateOASIS collection, ensuring the medication profile remains current,ensuring lab values have been reported to the physician timely,attending weekly case conference and monthly case manager meetingsare key responsibilities of this role.
• This part-time salariedposition works 24 hours/week, over 4 days in the week, and iseligible for our 60% paid days off program.

Qualifications Registered Nurses (RNs) must meet the following requirements: Be currently licensed as a Registered Nurse (RN) in the stateof employment A minimum of one year of clinical experience is preferred Demonstrate knowledge and skill in current nursingpractice Possess a valid state driver’s license and automobile liabilityinsurance Additional Information Enhabit Home Health & Hospice is an equal opportunity employer.
