As a hospice and palliative care nurse practitioner atAgape Care, you’ll provide patients with diagnosis, treatment, andcare under the supervision of a physician. In this role, you’llfocus on providing palliative care. Most importantly, you’ll assistfamilies along their health care journeys with love and compassion.

Qualifications:

A heart to serve patients and familiesand a passion for providing the best possible care

Education:

Graduate of an accredited Certificate, Diploma, Associate orBaccalaureate School of Nursing.

Experience:

2 years experiencewithin the last 5 years in chronic disease management/long-termcare. Hospice background preferred.

Required:

Reliabletransportation. Ability to sit, stand, bend, move intermittentlyand lift at least 25lbs.
