Our Crescent Hospice team is looking for a PRN Registered NurseCase Manager (RNCM) that is ready to make a meaningful differencein the lives of the patients and families we serve in and aroundthe Beaufort Area! We are looking for a high-energy, compassionate, detail-orientedRegistered Nurse (RN) to be a Hospice Case Manager for ourpatients. Our Hospice RN Case Managers plan, organize, and directhospice care utilizing the nursing process of assessment planning,interventions, implementation, and evaluation; and effectivelyinteract with patients, caregivers, families, and otherinterdisciplinary team members whiles maintaining standards ofprofessional nursing and clinical competency. Qualifications:

Hold current unencumbered license as a Registered Nurse. Minimumone (1) year experience as an RN in a medical surgical/acute caresetting. Hospice exp a plus!

Must be computer proficient in typingand various programs, including background in EMR.Possess andmaintains current CPR certification if required by state.$33.00 -$38.00 per hour (Average Pay Range). The pay range listedrepresents a general posting guideline for the role and is not afixed offer. Final compensation will be determined based on thecandidate’s relevant experience, qualifications, and the specificresponsibilities of the position. The exact compensation rate willbe discussed and confirmed at the conclusion of the interviewprocess.
