Provides comprehensive on-going case management services topatients by coordinating and managing the care of patients to meetmultiple service needs across the continuum of care. Ensure optimalpatient outcomes that address quality, service, customersatisfaction and cost-effectiveness. The Case Manager will partnerwith the multidisciplinary team to establish care and allocateresources associated with the patient’s risk assessment. Assiststhe patient/patient’s family in coping with illness by optimizingthe patient’s/family’s self-care abilities and supporting theirconsumer rights.

- The Nurse Case Manager (NCM) I reports to theManager of Case Management and Care Transitions.
Under guidance ofthe Nurse Case Manager Team Leader, the Nurse Case Manager Ifunctions as a member of the clinical team. This role coordinates aplan of care to achieve clinical outcomes and promotes a safe andtimely discharge. This position manages the plan of care throughoutthe care continuum to ensure the efficient movement of the patientthrough the hospital system. The Nurse Case Manager I serves as aclinical expert related to discharge planning, resourceutilization, and case management.
Minimum Training and Education:
- Bachelor of Nursing degreepreferred with minimum of one year of experience. RN staff hired onor after July 1, 2013 with an Associate or Diploma degree innursing are required to be enrolled in an accredited BSN programwithin two years and successfully obtain a BSN degree within fouryears of the RN hire or reclassification date. Refer to policy A141for more details.
- Prior leadership experience preferred.
One yearof case management and/or utilization review work experiencepreferred.
Required Licensure, Certifications, Registrations:
- Case Managementcertification preferred.
Licensure as a registered nurse by thestate of South Carolina or compact state required. Current BasicLife Support (BLS) or ACLS required, either a certification from anAmerican Heart Association (AHA) BLS for Healthcare Providers (orAHA recognized equivalent) or an American Red Cross CPR/AED forProfessional Rescuer and Healthcare Provider. Certification in CaseManagement preferred.
