PURPOSE AND SCOPE

RN CAP 1
is an entry level designation into the ClinicalAdvancement Program (CAP). This position is accountable andresponsible for the provision and coordination of clinicallycompetent care including assessment, planning, intervention andevaluation for an assigned group of patients. Assesses and managespatients’ response to home dialysis training and treatment therapyby following prescribed predetermined protocols and communicatespatient related issues to the physician as needed. As a member ofthe End Stage Kidney Disease (ESKD) health care team, this positionparticipates in decision-making, teaching, leadership functions,and quality improvement activities that enhance patient careoutcomes and facility operations.
PRINCIPAL DUTIES ANDRESPONSIBILITIES
All duties and responsibilities are expected to beperformed in accordance with Fresenius Kidney Care policy,procedures, standards of nursing practice, state and federalregulations.
- Performs all essential functions under the directionof the Supervisor with guidance from the Educator, Preceptor or incollaboration with another Registered Nurse.
- Performs ongoing,systematic collection and analysis of dialysis data for assignedpatients and documents in the patient medical record, makesadjustments or modifications to treatment plan as indicated andnotifies Supervisor or physician as needed.
- Assesses,collaborates and documents patient/family’s basic learning needs toprovide initial and ongoing education to patients and family.
- Directs and provides, in collaboration with the patient, home carepartner, direct and ancillary patient care staff, all aspects ofthe provision of safe and effective delivery of dialysis therapy toassigned patients.
- Administers medications as prescribed or inaccordance with approved algorithm(s), and documents appropriatemedical justification and effectiveness.
- Initiates or assistswith emergency response measures.
- Serves as a resource for healthcare team, participates in staff training and orientation of newstaff as assigned.
- Ensures correct laboratory collection,processing and shipping procedures are performed and reschedulesmissed or insufficient laboratory collections.
- Identifiesexpected outcomes, documents and updates the nursing assessment andplan of care for assigned patients through collaboration with theInterdisciplinary Team.
- Ensures patient awareness related totransplant and treatment modality options.
- Assists in theidentification, evaluation, selection and education of HomeDialysis candidates and Home Partners.
- Performs assessment andidentifies barriers of the Home Dialysis candidate’s homeenvironment and partner / family readiness and ability to performdialysis treatments in the home.
- Trains Home Dialysis patientsand / or Home Partners on the safe, effective operation andmaintenance of all Home Dialysis equipment and treatment suppliesthrough an organized and formal Home Dialysis Training Program.
- Required to complete CAP requirements to advance.
- Performs allother duties as assigned by Supervisor.

- The position provides direct patient care that regularly involvesheavy lifting, moving of equipment, patients and assisting withambulation. Equipment aids and/or coworkers may provide assistance
- This position requires frequent, prolonged periods of standingand the employee must be able to bend over.
- The employee mayoccasionally be required to move, with assistance, machines andequipment of up to 200 lbs., and may lift chemical and watersolutions of up to 30 lbs. as high as 5 feet.
- The workenvironment is characteristic of a health care facility with airtemperature control and moderate noise levels.
- May be exposed toinfectious and contagious diseases/materials.
- Rotates coveragewith other licensed home therapies staff as assigned to ensurereliable and adequate coverage.
- Position requires participationin on-call rotation, night, weekend, holiday or as defined byindividual program needs.
- The position may require travel totraining sites, other facilities or patient homes.
- May be askedto provide essential functions of this position in other locationsincluding patient’s home with the same physical demands and workingconditions as described above.
- Day to day work includes deskwork, computer work, interaction with patients, facility/hospitalstaff and physicians.
SUPERVISION
Assigned oversight of LPNs/LVNs,RNs, Patient Care Technicians and Home Therapy Care Team Assistantsas a Team Leader or designated Nurse in charge, after meeting allthe following:
- Successful completion of all FKC education andtraining requirements for new employees.
- Must have a minimum of12 months experience as a RN.
- Successful completion of 3 monthsexperience as a RN in home peritoneal dialysis and / orhemodialysis.
EDUCATION and
LICENSURE
- Graduate of an accreditedSchool of Nursing.
- Current appropriate state licensure.
- Currentor successful completion of CPR BLS Certification
- Must meet thepractice requirements in the state in which he or she isemployed.
EXPERIENCE AND REQUIRED SKILLS
- Entry level for RNs withless than 2 years of Nephrology Nursing experience as a RegisteredNurse.
- Minimum of 1-year experience as a Registered Nurse(preferred)
- Home dialysis therapy experience(preferred).
