Responsibilities Develop, coordinate, and update individualized plans of care, ensuring continuity and collaboration with the interdisciplinary team (IDG) and other staff. Conduct initial and ongoing comprehensive assessments, including physical exams, medical history reviews, and environmental evaluations to determine patient and family needs. Provide nursing services, treatments, and preventive procedures requiring specialized skills as ordered by the physician. Monitor and document changes in patient condition, pain/symptom management, and reactions to treatments, ensuring timely communication with the care team and physicians. Educate patients and families on care plans, hospice services, and health-related topics to empower informed decision-making. Supervise and evaluate the care provided by Hospice Aides and Licensed Practical Nurses (LPNs), ensuring compliance with care plans and conducting regular supervisory visits. Coordinate referrals to other hospice care disciplines and document rationale if referrals are not made. Perform on-call responsibilities to address patient and family needs during assigned times, including emergency care. Maintain accurate, timely, and detailed patient records in compliance with agency policies and regulatory standards. Participate in staff development meetings and contribute to the ongoing improvement of care delivery and patient outcomes. Qualifications:
Graduate from an accredited School of Nursing. Current licensure as a Registered Nurse (RN) in the state. Strong assessment and problem-solving skills. Proficient in organization, time management, and understanding case management processes. Valid driver’s license with access to a reliable, insured vehicle in good working condition. Excellent verbal and written communication skills. Clear RN license with no restrictions; must be able to administer medications, perform medication reconciliations, and provide nursing procedures or treatments within the scope of state nursing practice.
