Home Health Nurse AJM Homecare Services Dillon, SC 29536 Job Summary We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their health and well-being. You will work collaboratively with other healthcare professionals to develop and implement individualized care plans, focusing on the unique needs of each patient. This position requires strong clinical skills and the ability to manage complex cases effectively. Responsibilities Conduct comprehensive assessments of patients’ health status and needs in a home setting. Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams. Provide skilled nursing care, including catheter care, medical documentation, and management of acute care conditions. Administer medications and treatments as prescribed by physicians. Educate patients and families on health management, disease prevention, and self-care techniques. Coordinate with case management teams to ensure continuity of care and appropriate follow-up services. Maintain accurate medical records and documentation in compliance with regulatory standards. Utilize neonatal ventilator equipment when necessary for pediatric patients. Collaborate with laboratory services for diagnostic testing as needed. Experience Current Registered Nurse (RN) license in the state of practice. Previous experience in home care or acute care settings is preferred. Familiarity with hospital protocols and procedures, particularly in a teaching hospital environment. Experience in case management or working with diverse patient populations is a plus. Proficiency in medical documentation practices is essential. Knowledge of mammography procedures is advantageous but not required.
