Provide triage services to prioritize patient needs in fast-paced environments. Collaborate with interdisciplinary teams to coordinate care for patients across various settings such as nursing homes, critical care units, and primary care facilities. Educate patients and their families about health management, treatment options, and preventive care strategies. Maintain accurate documentation of patient care activities in compliance with healthcare regulations. Requirements Valid Registered Nurse (RN) license in the state of practice. Experience in long-term care, inpatient settings, or behavioral health preferred. Knowledge of emergency medicine practices and critical care protocols is a plus. Familiarity with ICD-9 coding is beneficial for documentation purposes. Strong communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work collaboratively within a team-oriented environment while demonstrating independence in decision-making.
