SUMMARY OFROLE
The MDS Coordinator (RN) plays an integral role in thesuccess of our team, our community and creating a place ourresidents are proud to call home. The primary purpose of thisposition is to conduct and coordinate the development andcompletion of the Resident Assessment Instrument (RAI) process inaccordance with the state and federal requirements and the policiesand goals of the facility.
Shifts/Hours:
Full-time | Monday -Friday, no weekends | 8am-5pm

Essential Job FunctionsConduct and coordinate the development and completion of theMinimum Data Set (MDS) assessment in accordance with current rules,regulations, and guidelines that govern the resident assessment,including the implementation and completion of Care AreaAssessment’s (CAA) and Care Area Trigger’s (CAT). Manages thereimbursement process and works collaboratively with the businessoffice and therapy department. Assist and provide input intoongoing updates of policies and procedures that govern thedevelopment, use, and implementation of the resident assessment(MDS) and care plan. Participate in facility surveys (inspections)made by authorized government agencies. Ensures facility is incompliance with the Resident Assessment Instrument (RAI) process.
Work with the Interdisciplinary Team (IDT) in completing acomprehensive MDS and care plan for each resident. Ensure that careprovided is in accordance with the resident’s wishes and Developand participate in the planning, conducting, and scheduling oftimely in-service training classes that assist in nursing to ensurethere is good supporting documentation for the MDS. Assist indeveloping any training activities needed concerning residentassessment/care plan skills. Coordinate the development of awritten plan of care (immediate and comprehensive) for eachresident that identifies the problems/needs of the resident,indicates the care to be given, goals to be accomplished, and whichstaff are responsible for each element of care. Monitor the careplan so that it includes measurable objectives and timetables tomeet the resident’s medical, nursing, and mental and psychosocialneeds as identified in the resident’s assessment. Assist theDirector of Nursing (DON) and relevant supervisors of otherdepartments in ensuring that all personnel involved in providingcare to the resident are aware of the resident’s care plan. Monitorand ensure compliance of the MDS process. Coordinate the review andrevision of the resident’s care plan by the interdisciplinary teamafter each quarterly review or other assessment, ensuring that thecare plan is evaluated and revised each time an assessment is doneor when there is a change in the resident’s status. PersonnelFunctions Perform employee performance evaluations (if applicable).Determine MDS staffing requirement, and making recommendations tothe DON concerning employee dismissals, transfers, etc. Review andevaluate your team’s work process and work to improve. Develop workassignments and/or assist in completing and performing suchassignments. Submit complaints and grievances DON. Knowledge andCritical Skills Be able to make independent decisions and followinstructions. Deal tactfully with personnel, prospects, residents,family members, visitors, government agencies/personnel, and thepublic. Capable of working with ill, disabled, elderly, andemotionally upset people within the facility. Communicateeffectively in a manner that is sufficient for supervisors, teammembers, prospects, residents, and families. Education andExperience Must meet all applicable state and federal requirementsfor this position. 1 year of experience in the RAI process. 2 yearslong term care facility care experience preferred. Possess acurrent and active license to practice as an RN in this state.
