Admissions Coordinator RN/Nurse Case Manager Hospice and Palliative Care of the Piedmont Greenwood, SC 29646 •

00 AM-7:00 PM•

Summary:

The Admissions Coordinator RN works cooperatively with the Home Care, Hospice House Nurse Manager and Palliative Care. Supervised by the Clinical Director of Hospice At Home Services.

Qualifications:

Current South Carolina license as a Registered Nurse, BSN preferred Willing to seek Hospice Certification after two (2) years of full-time service Minimum of 3 years of experience in a clinical position Ability to assess and respond to complex needs of terminally ill patients and their families Skilled in nursing practice, able to provide emotional support and acceptable of individual lifestyles Skilled in pain and symptom management Professional, positive attitude Strong communication and time management skills Ability to work as part of a team

Responsibilities:

Responsible for managing referrals in cooperation with the Home Care Manager, Hospice House Managers and Palliative Care Team. Works cooperatively with the Home Care and Hospice House Managers to implement the Plan of Care for each patient admitted to our program. Evaluates all referrals for appropriateness. Admits and assesses those eligible referrals. Documents in detail reasons for patients not admitted and if follow up is needed. Informs referral source of non-admit and plan, if any. If patient referred to other agencies, provides documentation. Assists with insurance company precertification and provides appropriate documentation, if needed, to ensure coverage for Hospice Home Care and Hospice House. Assists with providing needed documentation to Medicaid for primary patients. Enters new admissions and referrals in MatrixCare in the absence of the Clinical Assistant. Monitors patient admission and discharge activities daily. Collaborates with Medical Director or designee, Admission Coordinator, and the IDT to initiate the development of the Plan of Care for our patients. Assures the needs of patients and families are addressed and that problems are documented and followed up on in a timely manner. Manage the care of any assigned patients by making visits to patients within the service area by providing nursing care which includes patient/family teaching and support. Communicates regarding the patient’s status with physician, team members and other agencies involved in the care of the patient. Adheres to call regulatory standards and agency policies set forth. Participates in the 24 hour on-call as scheduled. Coordinates the transition of patient and changes in level of care Provides direct regularly scheduled supervision of the Nursing and CNA staff. Makes home visits as requested. Participates on committees as requested. Assists Home Care and Inpatient Managers as time allows. Attends IDT meetings and may serve as the IDT master in the absence of the Home Care Nurse Manager and/or the Hospice House Nurse Manager. Provides monthly quality and statistical reports. Supports the overall culture of the organization. Participates in organizational sponsored events. Performs related duties as assigned or as needs arise.
