QUALIFICATIONS & EXPERIENCEREQUIREMENTS
Graduate of an accredited school of nursing; RNValid RNlicense in the state employedThree years of experience in a longterm care environment preferredExperience with the MDS/RAI processand/or case management preferredJOB RESPONSIBILITIESThe MDS NurseRAC (Resident Assessment Coordinator) reports to the ExecutiveDirector and is responsible for accurate and timely completion ofmds assessments and coordination of the RAI process. ProvidesMedicare, Medicaid (case mix), and managed care oversight to ensureappropriate clinical services are provided and appropriatereimbursement is received for each resident. This includes ensuringthat the centers are in compliance with federal and stateregulations as well as the CommuniCare Family of Companiesguidelines and policies and procedures. This role serves as a keymember of the facilitys management team in helping the facilityobtain/maintain quality outcomes.
