JOB PURPOSE

To provide and coordinate the provision ofskilled nursing care under physician supervision, through competentapplication of nursing process.

KEY RESPONSIBILITIES
  • Providescompetent nursing care & interventions to home health patientsaccording to state and industry and profession
  • Supervises careprovided by Home Health Aides, Licensed Practical Nurses (LPN)assigned to the care team (completing supervisory visits of HHAs asrequired)
  • Helps maintain continuity of care by collaboratingwith the interdisciplinary team and attending physician to develop,integrate, and manage the patient care plan of care, makingreferrals to other disciplines as needed
  • Demonstratesintermediate knowledge of home care and competency in disciplinefor specific patient care skills, required for the provision ofcare
  • Conducts field visits to (1) evaluate & initiate start ofcare and (2) conduct clinical outcomes monitoring, follow-up andcare performance improvement initiatives
  • Utilizes acceptedcompany teaching materials in patient/family education
  • Assesses, monitors, documents and reports progress of patient’shealth and condition using required documents via clinicianpreferred technology, and within established timelines
  • Completes documentation timely, accurately, and at the point ofcare, according to industry standards (e.g., OASIS assessments,SOC/Admissions, Recertifications, ABN’s, subsequent visits,physician orders, care coordination, etc.)
  • Honorspatients/residents’ rights to be notified in advance of care,treatment and services to be provided and works with patient teamto adhere to physician-approved care plan. Ensures equitabletreatment, self-determination, individuality, privacy, property andcivil rights.
  • Complies with corporate compliance program
  • Meets agency productivity standards and utilizes time and resourceseffectively and efficiently
  • Participates in center/agencysurveys (Licensure/Joint Commission), mandatory in-services, caseconferences, staff meetings, and programs (e.g., PerformanceImprovement Program) as required
  • Follow established safetyregulations and procedures, and report job-related functions/tasksthat involve occupational hazards, as necessary
  • Maintainscurrent license, certification, and clinical records to meetfederal, state and agency regulations and guidelines
  • Ability tobe self-directed, prioritize, manage time appropriately, and meetoutcomes and strategic goals by targeted deadlines. MINIMUM EDUCATION REQUIRED
  • Nursing degree from an accreditedcollege or university, or graduate of an accredited/approved RNprogram.
MINIMUM EXPERIENCE REQUIRED
  • Minimum of one (1) yearof RN experience
  • Valid driver’s license
  • Reliabletransportation
  • Liability insurance to complete home visits
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW
  • Current,active, and unrestricted Registered Nurse (RN) licensure in stateof practice.
  • Current CPR certification.
