  • A Brief Overview
  • The Case Manager is primarily responsible for the coordination of resources and services in response to individual patient needs and fiscal realities.

As a member of the patient care team, the case manager is responsible for tracking clinical service utilization of specific patients from admitting or discharging. Tracking includes the concurrent collection of data, and analysis for appropriateness and timing of interventions. The Case Manager provides practical interpretation of the analysis for nurses, multi-disciplinary teams, medical staff and managers as the findings and recommendations relate to the individual patient’s outcomes, the health plan coverage, and the overall costs for both the patient and the hospital. Guided by the current disease state practices, the case manager is responsible for impacting the timely delivery of services, the length of stay and the appropriateness of treatment while supporting the triple aim of improving the patient experience, improving patient outcomes, and effectively managing the cost of care.

  • + Complete Readmission reviews + Complete all Medicare Medicaid and Commercial Reviews + Communicate with patients and their families about a patient’s health condition + Promote quality, cost-effective care, and patient outcomes + Plan and coordinate care, resources, and services on a case-by-case basis with family members, medical professionals, and other health service providers + Advocate for individualized options that meet a patient’s specific health needs •Education Qualifications•+ Associate’s Degree in Nursing (ADN) Required + Bachelor’s Degree in Nursing (BSN) Preferred •Experience Qualifications•+ Minimum two years RN Case management experience in health care delivery acute care setting Preferred •Skills and Abilities•+ Able to work independently, adjust priorities often and establish rapport quickly required + Exception critical thinking and problem skills required.

+ Demonstrated experience in diplomacy and ability to contribute to team goal setting and program development required. + Proven communication skills, both verbal and written required. + Expertise in facilitation of dysfunctional family and patient placement outcomes required. + Basic computer knowledge and skills required to manage multiple databases required. + Knowledge in acute care level psychosocial intervention and practices required + Knowledge with Case Management and outcomes required.

  • Licenses and Certifications
  • + Registered Nurse RN license in good standing SC with SCLLR. Required
  • PAT Rating
