Required Qualifications:

One year of recentexperience as a Registered Nurse, including a minimum of six monthsin a Home Health setting as an RN Proficient in OASIS documentationValid RN license in compliance with state regulations BLScertification (AHA/ARC)

Preferred Qualifications:

Home HealthNursing Certification (RN-BC) Other certifications and licenses maybe required for this position

Summary:

A Home Health RegisteredNurse (RN) delivers direct nursing care to patients in their homes,ensuring comfort, stability, and effective management of theirmedical conditions. Home Health RNs assess patient conditions,develop care plans, administer treatments, and educate patients andcaregivers on disease management.

Essential Work Functions:

Assesspatient conditions and develop individualized care plans based onmedical needs and physician directives Administer medications,treatments, and wound care while monitoring patient responsesProvide patient and caregiver education on medication management,disease progression, and self-care techniques Monitor for changesin patient status, reporting concerns and adjusting care plans asneeded Coordinate with physicians, therapists, and other healthcareprofessionals to ensure seamless care transitions Document patientcare, interventions, and progress accurately and in compliance withregulatory standards Ensure compliance with infection control,safety, and home care best practices to protect patients andcaregivers Support families and caregivers, offering guidance andemotional support throughout the care process Maintaincommunication with case managers and interdisciplinary teams toprovide coordinated patient care Strong ability to completecharting and documentation requirements while meeting visit andproductivity goals Adhere to infection control, safety, andhealthcare regulations Perform other duties as assigned within thescope of practice

Required Essential Skills:

• Remain in a stationary position, move about, moveequipment (50-100lbs), pushing, pulling, bending InterpersonalSkills
• To work effectively with a variety of personnel(professional and ancillary) to present a positive attitude andprofessionalism Technical/Motor Skills
• Ability to grasp, finemanipulation, pushing/pulling and able to move about when assistingwith procedures and/or using department equipment. MentalRequirements
• Must be able to cope with frequent contact with thegeneral public, customers, and meeting deadlines under pressure.
