Job SummaryA Diabetes and Nutrition Program RN Care Coordinatoris responsible for providing comprehensive care management,education, and support to patients with diabetes andnutrition-related health concerns. This role combines clinicalnursing expertise with care coordination and patient educationskills. The Nurse Care Coordinator will collect and analyzepatient-level data, assist with the development and maintenance ofdiabetes and nutrition care plans, and evaluate outcomes ofinterventions. Assures compliance with regulatory body standards(including Joint Commission, HRSA, PCMH, and grant fundingsources).Major Duties And ResponsibilitiesAnalytical and CriticalThinking Assess patients’ nutritional and health needs usingdocumentation in the Electronic Health Record Conduct an interview and assess patient’s understanding ofdiabetes and nutrition knowledge to prepare an individualized careplan. Collect and analyze performance improvement dataCommunication Counsel patients on nutrition issues, healthy eating habits,and diabetes self-management Provide education on medical conditions, including diabetes,through various formats such as group classes, workshops, andwritten materials Provide a basic diabetes and nutrition education and coordinatefollow up for Diabetes Self-Management Training (DSMT), nutritionmanagement or pharmacotherapy management. Participate in strategic planning and operational improvementinitiatives Provide training and orientation for clinical staff on diabetescare and nutrition management Train and supervise diabetes and nutrition navigator toefficiently support the program operations at its maxcapacityTeamwork Collaborate with interdisciplinary healthcare teams to ensurecontinuity of care Coordinate transitions of care and referrals to internal andexternal services Serve as a resource for Chase Brexton staff on nutritionalmanagement of patientsCompliance, Policy, and Procedure Drive compliance to the National Standards for DiabetesSelf-Management Education and Support (DSMES) Develop and implement policies and procedures related todiabetes care and nutrition servicesWillingness to Learn Engage in professional development activities to keep abreastof care management practices and patient engagementstrategies. Attend training sessions and staff meetings as assigned.
SkillsAnd Abilities Ability to coordinate communication effectively between variousdisciplines involved in diabetes and nutrition management. Able to prioritize competing responsibilities and managecomplex caseloads. Motivational interviewing skills. Experience with documentation in an Electronic MedicalRecord Ability to work with computers in word processing, and databaseapplications preferred. CPR certified by scheduled start date. Must be flexible and adapt to a changing environment.
EducationAnd/Or Experience Graduate from an Accredited School of Nursing; Current MarylandNursing License Chronic disease management experience, especially inDiabetes. Encouraged to maintain membership in professional organizationssuch as the AAACN, ANAC, etc.

Certified Diabetes Educator (CDE) or CertifiedDiabetes Care and Education Specialist (CDCES)WorkingConditions/Physical Demands Work is typically performed in an office environment.
