Leidos-QTC Health Services has a Medical Case Manager openingfor an experienced Case Manager to join our team supporting theOccupational Health services for an Intelligence Community customerlocated in Chantilly, Virginia. this role will be be employed under”Jamshid Tamiry, M.D., P.C., dba QTC Medical Group of Alaska, aLeidos QTC Health Services affiliated medical professionalcorporation” Candidates must already possess Top Secret clearancewith polygraph. Case Manger Duties and Responsibilities Provide Case Management services for injured, ill or recoveringclients. Provide clinical assessment, planning, implementation,coordination, monitoring and evaluation of the options and servicesrequired to meet the needs of clients and their familymembers. Accurately collect and record utilization metrics and providemonthly reporting Assess requirements for durable medical equipment, physicaltherapy, occupational and rehabilitative services for clients Utilize electronic medical records and telemedicinetechnologies on behalf of clients and their family members Be a skilled communicator and able to assist the client inunderstanding the administrative requirements needed forreimbursements To communicate effectively in written and spoken word with theteam and community providers Formulate timely case reports, to include presentation offactual information and analysis of such Perform ad hoc duties as per the Program Manager or sitesupervisor’s discretion, including but not limited to research,policy and procedure development, briefing leadership on casestatuses.

RequiredEducation and Experience:

Education:

Bachelors degree in Nursing or Medically RelatedField Licenses (must have one): Registered Nurse, or NursePractitioner, or Physician Assistant License must be in Virginia or from a

Compact LicenseState Experience:

Must have five (5) or more years previous Case Managerexperience preferably in military treatment facilities, home healthcare, and/or mental health facilities Experience with care delivery, psychosocial support andreimbursement systems. An understanding of rehabilitative strategies. Previous experience in writing factual, clear, and concisereports, to include situational analysis Ability to maintain daily scheduled activities and effectivetime management skills

Security Clearance:

Applicants already possess Top Secretgovernment clearance with polygraph prior to start PreferredQualifications Posses current board-certification as a Certified Case Manager(CCM) from the Commission for Case Management Certification. Previous experience working with government as contractor orstaff member

Pay and Benefits:
Pay Range:

$54-60/hr.
