Key Responsibilities:
Identify and address patient symptoms through evidence-based interventions Educate family members about care processes Document clinical findings and treatment updates carefully Collaborate in care meetings and care coordination Serve as a resource for staff education
Requirements:
APRN licensure and DEA credentialing 2 years of relevant clinical practice Physically able to perform patient support tasks
