Key Responsibilities:
Conduct physical exams and evaluate palliative needs Collaborate in care plan development and adjustment Provide end-of-life education and resources for families Maintain regulatory compliance and detailed charting Attend and participate in care planning discussions
Requirements:
Certified Nurse Practitioner with DEA registration At least 2 years’ experience in palliative or chronic disease care Ability to drive and assist in basic caregiving tasks
