We’re hiring a compassionate Nurse Practitioner to provide expert care for individuals facing life-limiting illnesses. This role emphasizes symptom management, comfort care, and emotional support for both patients and their families.
Key Responsibilities:
Perform patient evaluations and symptom assessments Collaborate with interdisciplinary teams to create and maintain individualized care plans Provide education to caregivers and families Maintain clinical documentation and compliance with care standards Attend and contribute to case conferences and care planning meetings
Requirements:
Graduate degree in nursing with Nurse Practitioner certification Active licensure and DEA credentials Minimum of 2 years’ experience in long-term or palliative care settings Ability to travel locally and lift/move patients as needed
