Key Responsibilities:
Manage symptom treatment plans and patient assessments Educate caregivers on disease progression and symptom management Document care thoroughly and update medical records Coordinate care across disciplines Support caregivers and families through emotional transitions
Requirements:
Active NP licensure and DEA authority 2+ years in palliative, hospice, or long-term care Physical fitness to assist with patient needs and travel
