Client Care Coordinator Locations:
Sumter, Columbia, Florence, Mt. Pleasant, Summerville, Fayetteville, and surrounding areas Comfort Keepers is seeking a compassionate and driven Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our care team as a Traveling Case Manager or Client Care Coordinator . This is a dynamic field-based role, providing clinical oversight and care coordination for in-home care clients across multiple South Carolina and North Carolina locations.
Pay Range:
$28 – $40 per hour (Rate dependent on licensure, experience, and location) Company vehicle provided for work-related travel
Position Overview:
As a member of our clinical team, you will ensure continuity of care, conduct client assessments, and supervise caregivers in delivering exceptional service to our clients. You’ll serve as a liaison between families, caregivers, and health professionals—playing a critical role in enhancing the quality of life for our clients.
Key Responsibilities:
Conduct in-home assessments and create/review care plans Provide ongoing supervision of caregivers and support staff Maintain regular client follow-ups and reassessments Train and evaluate caregiver skills Coordinate care with families, providers, and agencies Participate in on-call rotation and case conferences
Qualifications:
