If you are interested in this position, please Apply or Email me with your resume attached at Jmahnke@alinestaffing.com Senior Care Manager (RN) Compensation The pay for this position is $45.00 – $50.00 per hour Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Senior Care Manager (RN) Highlights The required availability for this position is Monday through Friday, 8:00am-5:00pm EST This is a remote position, but candidates must reside in South Carolina Some travel (up to 25%) is required to visit members Assignment is 4 months with the possibility to extend or convert Senior Care Manager (RN) Responsibilities Provide transition of care case management services for Medicaid members after hospital discharge Perform care assessments and help close HEDIS care gaps to support STAR ratings Coordinate and deliver remote case management services via phone and electronic communication Track and document outreach activities in accordance with compliance guidelines Collaborate with other team members in a highly communicative and mission-driven environment Ensure member contact is made within 10 business days post-discharge Senior Care Manager (RN) Requirements Must reside in South Carolina and be willing to travel up to 25% Licensed Health Provider – RN, LCSW, LPC, LMFT, PA (certified in psychiatry), Psychologist, or other HEDIS-qualifying provider 2+ years of clinical experience, preferably in psychiatric or mental health care Strong computer and documentation system skills Ability to meet strict performance and outreach timelines Senior Care Manager (RN) Preferences Bachelor’s degree required , Master’s degree preferred Experience with Medicaid and case management Proficient in Microsoft Office , Teams, and care coordination platforms If you think this position is a good fit for you, please apply to this posting!
