To ensure we deliver the right care from the start, we’re seeking Licensed Nurses (RNs & LPNs) to conduct initial and follow-up assessments . About the

Role:

As a Nurse Assessor , you’ll be the first set of clinical eyes on the client. Your insights are vital in helping us create safe and effective care plans. This is an assessment-only role—no hands-on care required.

Your responsibilities will include:

Conducting initial in-home assessments for new clients Evaluating the client’s overall health and home environment Documenting findings to help shape personalized care plans Performing routine reassessments (30, 60, 90, and 120-day intervals) Providing monthly wellness check-ins for ongoing clients

What You Bring:

A current RN or LPN license (South Carolina) Compassion, professionalism, and attention to detail Reliable transportation to visit clients across different counties A desire to make a meaningful impact—without hands-on responsibilities

Note:

RNs may assess both adults and children LPNs may assess adults only , per state guidelines

Pay Structure:

$35 per assessment (driving distance: 1-30 minutes) $50 per assessment (31-60 minutes) $75 per assessment (over 1 hour of travel) We value your time, expertise, and the care you bring to every visit.
