Hospice & Community Care of Rock Hill, SC is actively hiring an RN Case Manager to work Full Time!

Shift Details:

Monday – Friday (8:00am – 4:30pm) Hospice & Community Care has been a cornerstone of compassionate end-of-life care in our community for nearly four decades. We are seeking an RN Case Manager to work Full Time to serve our community in and around the Rock Hill, South Carolina region. Our organization is fully dedicated to meeting the needs of patients and families, and helping them receive the care they need.

What You’ll Do:

Assess hospice patients, coordinate and implement care plans, and evaluate interventions for hospice patients. Lead hospice care team members in coordinating services for a caseload of patients in accordance with the physician. Provide outstanding nursing care, including teaching and supporting the patient as well as the caregiver. Care is provided through in-person visits and telehealth support. Present services to potential patients, obtain initial physician orders, and complete all necessary paperwork related to nursing admissions and other visits as necessary.

What You’ll Need:

Registered Nursing license in South Carolina, or be eligible for licensure in South Carolina. Valid CPR certification Experience in healthcare, home care, or hospice setting Key Benefits of Joining our
