Utilizes the nursing process to plan,implement, and evaluate patient/client care and/or servicesprovided to client(s) receiving contracted services, asrequiredInitiates the plan of care when directed and requests fromprescriber any necessary revisions as patient status and needschangeIncorporates therapeutic, preventive, and rehabilitativenursing actions based on patient diagnosis and establishedgoalsIntervenes as applicable in response to changes inpatient/client condition and reports such changes to appropriatephysician or supervisorAdministers medications and treatments asprescribed, as well as, monitors and documents effectiveness ofinterventionReviews plan of care and program protocols andidentifies areas for healthcare teaching/education, carecoordination, goal setting and discharge planning needs, asapplicable

Requirements:

Current Registered Nurse (RN) License forthe state in which the nurse practices.

Current Health Certificate(per facility Registered Nurse (RN) contract or stateregulation).Current PPD or Chest X-Ray.

Current BLS card.
